QUALIFICATIONS/HEALTH CLEARANCE (Cosmetic tattooing only)

• Age requirements: 18-60 years old. Please send photos of brows if you are over 60 and list any medication or health conditions.
• Must not have any transmittable blood related conditions or diseases such as Hepatitis or HIV/AIDS
• Must not be pregnant or nursing
• Must not have any skin allergies to pigments or makeup
• Must not be taking any blood thinning medication
• Must not have skin concerns such as eczema, psoriasis, or keloids in the treatment area
• Must not have undiagnosed rashes or blisters in brow or lip area
• Must not have experience with post-inflammatory hyper-pigmentation
• Must not be someone who is epileptic and has experienced a faint spell or seizure
• Must not have used accutane in the last 12 months
• Must not have active cancer
• If you are diabetic, please have a doctor’s note with medication + controlled diet
